District Health Centre (DHC) aims to provide primary healthcare services including health promotion,
assessment and patient empowerment program to enhance the public awareness on disease prevention
and the capacity in the self-management of health through the public-private partnership.
Community Health Officer (Eastern District Health Centre)

Key responsibilities:
Deliver health education activities and community programmes for promoting health awareness,
disease prevention as well as the importance of peer support and social inclusion in DHC
Deliver community rehabilitation & self-management programs for attaining well-being of
persons with disability, chronic illness and their caregivers

Provide service to facilitate mutual support groups and volunteer groups

Assist in providing community health or rehabilitation support programs
Requirements:

A recognized Bachelor degree in Public health, Community health or other health related degree
br>
1-2 years of experience in working in the community, primary healthcare or serving for people
with disability, chronic illness, their caregivers and related community organizations is preferred

Knowledge in public health, exercise, health management, chronic disease management, mutual
support service, volunteer service and e-service delivery
br>
Inter-personal skills, communication skills, time management skills
br>
Mature, self-initiative and innovative
br>
Good team player in a multidisciplinary team
br>
Good command of both Chinese and English
br>
Shift duty including 1-2 days night shift until 9:00 p.m. per week is required
Work Location: Eastern District
The successful candidate will normally be appointed on contract terms. Renewal of contract will be
subject to the service/operational needs of the department and the candidate's performance.

- Registered Social Worker qualification is an advantage but not mandatory.
- Good knowledge of service delivery and case/project management and familiarity with support services for chronic diseases is an advantage.
- Strong leadership, people management and coordination skills with the ability to motivate and build a high-performing team.
- Good analytical, coordination and interpersonal skills, mature and able to work independently and under pressure.
- Mature personality, strategic minded with vision, strong interpersonal and leadership skills.
- Excellent command of both written and oral English, Chinese/Cantonese/Putonghua.
