Founded in 1936, the Boys’ and Girls’ Clubs Association is one of the longest serving non-government organizations dedicated to the welfare of children and youth in Hong Kong. To support future development of our Association, we are now inviting competent candidate to join our team:

Responsibilities

  • Design service strategies based on school/ community needs as assessed
  • Implement the work plan and to evaluate the effectiveness of the group, programs and case intervention for quality improvement
  • Provide information and consultation to youth, parents and the community to utilize community resources to meet their needs
  • Coordinate with other organizations and disciplines for resources to support the students’ development
  • Prepare and submit up-to-date case reports, program proposal, budget, evaluation, financial and statistical reports on activities and programs
  • Control program expenses within budget and handle receipts in accordance with accounts procedure
  • Perform ad hoc duties as assigned
  • Working Location : Hong Kong West District

Requirements

  • Bachelor Degree or above in Social Work
  • Registered Social Worker
  • Good command of oral and written Chinese and English is necessary
  • Responsible, self-motivated and able to work independently under pressure
  • 2 years’ experience in Social Work and school related works are an advantage

General Job description:

This is a full-time on-site role located in Hong Kong SAR for a Case Management Nurse.
Our Nurses function as the driving force of the medical team, striving to help ensure our patients get the right diagnosis and implement the best treatment options and creating personalized medical solutions.

Main Responsibilities:

• Obtaining and organizing all medical documents and information needed to manage the case. Reviewing all the medical information to deeply understand the case.
• Coordinating all parties involved in the medical case: the Patient and his/her family; the Case Manager Doctor; the medical specialists locally; and other relevant medical parties. Follow up and handle all medical needs. Run meetings and phone calls, ensuring the patient medical management is executed in the best possible way.
• Overseeing the execution and outcome of the clinical plan as developed by Medix’ multidisciplinary team.
• Continuously update the Personal Medical Case Doctor regarding the status of the medical cases, as well as raising any problem or special situation in need of immediate attention to the Global Medical Director. • Establish a close relationship with the Patient and his/her family and be fully acquainted with his/her medical case. Be a positive driver in successful participation and work according to the highest service level agreements. • Assisting the Case Manager Doctors in the management of the cases and the Global Medical Director in the quality control process.
• Proactively identifying and monitoring the Patient and his/her medical condition and overall well-being: support in symptoms and complications management and care coordination support.
• Emotionally supporting the Patient and his/her family as they cope with the challenges presented by dealing with a significant medical journey.
• Referring and supporting patients with the management of their emotional stressand overall mental wellbeing as they cope with a serious medical condition.
• Documenting all interactions and creating the necessary clinical reports in a computerized case management system as well as a CRM system. Confirming that the Patient receives and understands any medical summaries, test results or recommendations.
• Assisting with the execution of Patient satisfaction surveys and preparing for submission to Care Quality Commission. Ensuring that all Medix policies and protocols are followed correctly.
• Explaining the exact nature and added values of our service, while assessing theneeds of the patient, understanding and managing the expectations of the Patient and/or his/her family.
• Ensuring that all Medix medical policies and protocols are followed correctly.
• Participate in meetings with the Global Medical Director with regards to medical challenges and opportunities and medical patient / case reporting and analysis of future needs.

Qualifications

  • Bachelor’s degree in Nursing or a related field
  • Valid nursing license in Hong Kong SAR
  • Registered nurse with years of in-ward working experience in public and/or private settings
  • Higher education or professional qualifications with less experience will also be considered
  • Passion for medicine and patient care
  • Excellent communication skills in Cantonese, Mandarin and English, both verbally and in written
  • Excellent Organizational skills
  • Assertive and proactive, professional
  • Strong writing skills & Computer proficiency

  • Report to the School Administrative Office, provide school-based counseling support for DAE students on individual as well as group basis;
  • Managing overall class operations, including maintaining communication channels, sharing official announcements, and addressing or redirecting student inquiries as needed.
  • Develop and implement strategies and prorgammes to support students in learning. Conduct assessments to identify students' needs and create personalized support plans;
  • Act as the person in charge to respond to students crisis situations in college in collaboration with the School Administrative Office. Collaborate with college staff to develop and implement crisis management protocols;
  • Work closely with teachers and School Administrative Office to address students' academic and social-emotional needs. Participate in multidisciplinary team meetings;
  • Prepare and submit up-to-date case reports, programme proposal, budget, evaluation, financial and statistical reports on activities and programmes;
  • Perform other ad hoc duties as assigned by supervisor.

  • Have Level 2 or above in English Language and Chinese Language in HKCEE / HKDSE or equivalent;
  • Good command of written and spoken English;
  • At least 2-3 years of social work experience;
  • Working day : 3-day a week
  • Working Period (Sep 2025 – May 2026)

Key Responsibilities

  • Act as the first point of contact for our community members across various platforms, providing timely and helpful support.

Qualifications
